Private Provider Program

1. General Information

The use of Private Providers is authorized by law in Section 553.791, Florida Statutes, Alternative Plans Review and Inspection. The Private Provider is contracted by the Owner to provide building code inspection services, but in technical matters is subject to the authority of the Building Official. Private Provider services may include inspections only, or plans review and inspections. The City of Coral Springs will not allow plans review only. The Private Provider’s role may be modified at any time by submitting a revised “NTBO”, subject to the restrictions set forth below.

An Owner may elect to use a Private Provider at any time. However, the Private Provider is recognized only after the City of Coral Springs reviews and accepts the “Notice to Building Official” (NTBO). All documentation must use official City of Coral Springs forms, except that inspection reports may show the Private Provider’s letterhead.

Private Provider’s role: Private Providers may review plans and perform inspections for the following disciplines only: Building, Roofing, Structural, Electrical, Mechanical and Plumbing. All other disciplines (Zoning, Fire, Public Works, etc.) are handled by the City.

Changing the Private Provider during plans review: If the Private Provider is terminated, or otherwise fails to complete the plan reviews and issue the appropriate affidavits, the Owner may find another Private Provider to finish the reviews and assume all responsibility. Alternatively, the City may take over the plan reviews, but will conduct a full re-review of all disciplines.

Changing the Private Provider during inspections: If the Private Provider is terminated, or otherwise fails to complete the inspections and issue the final compliance documents, the Owner must secure another Private Provider to take over the inspection duties until the completion of the project. The City of Coral Springs will not take over inspection duties.

Fee adjustments for building permits: A reduction in permit fees may apply, as follows: For plans review and inspections, the building department portion of the permit fees will be reduced by up to one third (30%). For inspections only (or if the City began to review the plans before the Private Provider election was finalized), the fees will be reduced by up to one half of one third (15%).

2. Guide to Using the Official Forms

The City of Coral Springs Building Department requires the use of our own forms for Private Provider related functions. All documents are designed to be in compliance with Chapter 553.791, F.S. “Alternative Plans Review and Inspection” regulating the use of Private Providers. To avoid unnecessary difficulties with our permitting and inspection processes, these documents must be executed properly. All required documents must be uploaded to the permit via our online portal. Copies of certain documents are required at the job site. Please read carefully, then select the appropriate document.

3. Election Documents

Required to elect a Private Provider, identify project personnel, authorize services, or acknowledge participation. These forms must be properly submitted and approved per project.

  • Form 098.A — Notice to Building Official (NTBO). The Notice to Building Official is executed by the fee owner or the fee owner's authorized representative, and officializes the use of a Private Provider. It specifies the scope of services to be preformed - either plans review and inspections, or inspections only. A separate NTBO is required for each master permit or stand-alone permit, but not for sub-permits or additional plans (shop drawings) which are associated with the master permit.
  • Form 098.A1 — Personnel Identification & Job Site Directory. This document identifies all Private Providers and Duly Authorized Personnel to be used on the project. A copy is to be posted at the job site during construction.
  • Form 098.A2 — Change of Private Provider Services. This document is required  to change from one Private Provider firm to another, or to terminate the Private Provider and opt for City of Coral Springs plan reviews/inspections (restrictions apply), or to change the services being performed by the Private Provider. A revised NTBO form is required.
  • Form 098.A3 - Private Provider Acknowledgment. This document outlines the duties and responsibilities of the Private Provider and provides useful information for plans submittal, inspections, and general procedures. The Private Provider's signature is required.
  • Form 098.A4 - Fee Owner's Authorization to Use Private Provider*. This document is required when the Private Provider is not retained by the property owner directly but by the owner's contractor or a tenant in the owner's building.

*NOTE: Forms 098.A, 098.A1, and 098.A3 are required for all projects. Forms 098.A2 and 098.A4 are submitted only when applicable.

4. Plans Review Documents

These B-series documents are required if Private Provider is to provide plans review and inspection services. The City of Coral Springs will not allow plans review only. These documents affirm that plans have been reviewed for code compliance by authorized professionals and are required before issuance of permits under Private Provider services.

  • Form 098.B1 - Plan Compliance Affidavit. This document is required if the plans are reviewed by the Private Provider to certify that the plans are in compliance with the technical codes. Each affidavit my represent only one review discipline.
  • Form 098.B2 - List of Approved Drawings. Use this form to list all individual drawing pages by drawing title and latest revision date. Accuracy is essential.

5. Inspection & Trade Close-out Documents

These C-series documents (in addition to Form 098.A1) are required if Private Provider is to provide Inspection Services only or Plan Review and Inspection Services. These documents must be completed and submitted at project milestones and final closeout.

  • Form 098.A1 - Personnel Identification & Job Site Directory. (Described above under Election Documents.)
  • Form 098.C1 - Inspection Report(s). Copy to be maintained at the job site and available at all times for verification by the Building Official.
  • Form 098.C2 - Trade Close-out Affidavit. To be used when closing out each inspection trade and submitted to each trade chief prior to the project closeout.

Note: Forms 098.C1 and 098.C2 are mandatory, but are designed to accommodate a custom logo and other desired information.

6. Compliance Documents

These D-series documents are required when submitting final certificates for project close-out.

  • Form 098.D1 - Certificate of Compliance (CO/CC). This document certifies that the project has been fully completed, all inspections have been approved, and that all required plan revisions and/or additional plans have been submitted to the City of Coral Springs and approved. This document is a formal request for the Building Official to issue the Certificate of Occupancy or Certificate of Completion.
  • Form 098.D2 - Certificate of Compliance (TCO/TCC). This document is used  for partially completed work, or for other situations where a standard CO/CC is not warranted. It must include a detailed explanation of any proposed exclusions or conditions in qualifying the building for certification. All is subject to evaluation by the Building Official prior to approval.

7. How to Upload Documents

All documents must be uploaded to the permit via our online portal.
